Summary

This document contains Referral Frameworks that can be adapted and developed locally, in negotiation with the local health services and health care professions in order to improve the referral process. The Frameworks are not prescriptive but are an illustration of good practice; they are there to stimulate discussion at local level. There are clear long-term benefits to patients and the health care system from reducing unnecessary referrals and ensuring that the referral process is effective. Collaboration at local level to ensure that this occurs in an optimal manner is vital.
