I am really struggling to get a good view on direct ophthalmocopy on patients with small pupils (and lens opacities).

I get as close as I can, use dim illumination and sometimes use the macular stop, but still get a poor view and so many reflections.

Please help!

You join the rest of us in struggling, sometimes looking from above or below will sneak you a view but realistically the patient probably needs dilating for a decent view.
