My supervisor has told me that I do not need to include visions on my case records if the patient wears their glasses all of the time. Is this correct?

I can think of a few cases where that would not be correct, mostly where unaided vision is required for decisions regarding suitability for occupation is required.

also if px calls in to say theyve lost their gls, u'd need visions to give appropriate advice eith regards to driving etc

I do it on everyone - but then, I am never caught out by my notes! I make sure they have all the information I will need for the current job, a audit trail, and also infomation for the unexpected future.
