30 prism dioptre constant RSOT without Rx and Ortho with Rx

 

Would you expect to have a greater difference in rx to get the RSOT or would you expect ALTSOT???

You need to do a cyclo refraction to make sure you have the MAX plus. Is your cover accurate make sure px def has phoria not tropia, after giving full rx. If unsure try bagolini or a stereopsis test to confirm binocularity.

Based on my experience within HES, I would say this is enough anisometropia to account for constant ET right eye. Sometimes there is more, but not always. As above, if you have done cyclo to confirm full plus Rx.

 

Another question which may come up in case records is whether there could be a residual microtropia when corrected, as there often is in these cases. If VAs are truly 6/5 either eye, this is unlikely, but if VA is even 6/6, 6/5, there may be a microtropia present. Check for stereopsis and 4 base out to confirm.
