how would someone manage a grade 2 seal and smile stain if wearing EW or monthly soft?

You have to address both problems before considering EW.

SEALS caused by mechanical interaction of lid on lens. Can be seen esp with stiff Si Hydrogel lense or standard soft. Need different design and/or material.

SMILE caused by dessication so maybe different material and use wetting drops. If not clear these patients may not do well with EW regime but can usually do ok on restricted wear times of daily wear modality.

Any epithelial insult contraindication to EW as bacteria adheres to lens and is more likely to infect.

SMILEs can be caused by incomplete blinking due to wearing of CL as well so sometimes all that is needed is Px education on how to blink properly!
