hi,does anyone know which are the LATEST codes for the documents required to register a Px as partially sighted or blindin england and wales? these are the ones that the ophthalmologist has to complete. can you let me know where can i find the info in the net. i tried coll of optom and rnib. they used to be called as BD8

CVI - cert of visual impairment which is completed by a reg consulatnt ophthalmologist. You also need to be aware of RVI - Referral of VI and i forgot the other one

The three forms are:

LVI- Letter of Visual Impairment

This is used by OO's to give to Pxs who they think may benfit from help from the social services - the OO does not have to sign it but his/her name is on it.

 

RVI- Referral of VI

This is given out by hospital staff where the Px may again benefit from social services help. The Px may not be eligible to be registered, but is entitled to help

 

CVI-Certificate of VI

used onl by Ophthalmologists - ie to certify the Px. Similar to the BD8 form but has a few more parts to it
