I had a lady today in her mid forties. Her corrected vision RE is 6/7.5 and her left eye 6/21. Two years ago her left eye had been its normal (amblyopic) 6/12. Both eyes looked healthy with no flare or retinal changes. Her complaint was that over the last month she had felt a pressure within the eyes and that her eyes were getting bad. Pressures 16 - open angles. Central fields right eye normal - left eye sl patchy (which could have been the amblyopia). However, confrontation showed right sided incongruent hemianopia with general bilateral field constriction (especially left eye). No headaches, no pupil defects, disks looked normal - no pallor or hyperaemia or neov. Further surprise was that while motility was normal - her eyes pained on looking upwards. I thought - okay it could be one thing - but how do I reconsile that with the field changes? She said her sinuses were normal and didnot feel ill. Any ideas appreciated.

this turned out to be what I feared - a pituitary tumur -
