Female px, 33 yrs old, wearing Acuvue Advance and using Cyclean.Presents after 8 weeks with slight vision reduction in left eye. Around 15 stromal infiltrates present. No discomfort/discharge. Advised no lens wear for 2 weeks. Reassessed and fitted with Oasys after infiltrates cleared. Reviewed 1 week. Infiltrates returned but fewer. Have advised no CLs again 2 weeks and will change solutionto Pres. Free. Since unilateral unlikely to be allergy, palpebral conj. clear.Your thoughts appreciated.

CL fit not adequate in that eye. Tear exchange poor in that eye with that lens. Infiltrates will disappear up to 2 months from start of management. Better off to issue some dailies in the meantime for 'desperate wear'. The flattest BC the better.

 

Don't think it is the solution.

Did you know that J&J recomends Optifree Xpress solution only with their new hydraclear lenses? Polyhex not very good!

Oh I rang them reg. this and they said cyclean was ok? I ring them again cheers.

Tear exchange is good.........looks like some form of recurrent viral problem but with minor/no symptoms.

I like acuvue adavance and oasys - bril lens. Sad abt the infiltrates - I had them once from a dose of adenose - took some time to clear and the periauricular node hurt !

Dily lenses till clear.

We use Renu+ with AA - no probs at all.

Ive seen a few cases of similar infiltrates.Sometimes they can be pretty stubborn.I think the big thing is not to revert back to FT use until the infiltrates are completely resolved.Restarting too soon can lead to a reccurrence and overall it may take many months to resolve.

As Palfi says PT use of dailies seems to be the short term answer to allow the patient to wear lenses.
