What sort of forms do people generally tend to use for private eye exam prescriptions?? Do you generally just get them printed by local printers?

I am currently getting little credit card sized card - a little like OE. Get is printed with your logo and colours. I think bigger ones like SS and Boots are too much.

yourself can do it with a good programme and a quality printer or your local printer, otherwise there is few a companies out there that could do the job but they are more expensive overall.( look at the directory or suppliers list)

A4 printed from a mail merge.

Looks good and costs little.

AOP do A5 card which allows for CL details as well.
