Need some suggestions please!

I have a lovely lady patient who was fitted with Night & Day 8.6 on a continuous wear basis 3 months ago. Absolutely adores her lenses and is desperate to carry on with them, they've changed her life etc.!

The problem is she's developed SEAL stains bilaterally. We previously tried purevision which weren't as comfortable and the movement on blink just wasn't adequate for me to be happy with it.

There never seems to be much info on the long-term effects of things likes SEALS - would I be woefully negligent in letting her continue? I would imagine such an epithelial breach is increasing her risk of serious corneal infection???

Or can anyone suggest a continuous wear lens that might be a good replacement (is the 8.4 likely to do the trick for example or will this worsen it?) - haven't really any experience yet of the acuvue CWs; have found Optix O2 brilliant but not really intended for regular overnight? I don't think the overnight element is the problem think it's just probably modulus-related.

Hi abs

Bilateral seals are a compromise in the corneal epithelium integrity - and yup : would increase the risk of MK (especially as she sleeps with them). Don't forget that the typical person with MK from CL use is a smart professional person, in early thirties - and not what you imagine! So best not to allow these to develop. The seals possibly due to fitting and the thickness on the N & D. Try an 8.40 - just check that the lens is still has sufficient movement to allow tear debris to flush from under the lens on a blink. This may work - sometimes things that work - go against all logic! J and J make a lens called an Acuvue Advance Oasys - this is a newish lens that is 'softer' and much more pliable that N & D or Purevision. It also has a much more slippery surface. It is designed for weekly overnight use : it will solve the problem in all likelihood. J & J make some bril lenses - look into them. The O2 Optix is little brother to N & D and is a good lens for daily use - but is only reccommended for odd over nights. Hope it helps - hey - how old is yr lady ? Let us know how you get along Palfi
