A few months ago, I had a patient in with a macula-off detachment. She was elderly, but not particularly myopic, and had no history of trauma or floaters, which surprised both me and the hospital. Visual acuity (VA) was down to hand movements. Her detachment had peeled off temporally, as they often do, and it got me wondering why she hadn’t noticed it until she closed the other eye and noticed that her vision was severely reduced.
