Creating a SMART action plan is a good way to make sure that any issues you identify acted upon effectively.  This might be, for example, outcomes of an audit you carry out, or gaps you’ve spotted in your knowledge that you need to address.

This action plan template can be used in a wide range of contexts and you can adapt the form as needed.

This guidance complements the online training and covers each stage of the process in more detail. It provides practice examples of successful audits which have been completed by optometrists.  It can be read in conjunction with the College guidance on clinical governance.

Quality in Optometry 
Provides useful audit resources for England and Wales including GOS checklists, and interactive audit toolkits (including record keeping, infection control and information governance audits).

A spreadsheet that you can adapt as needed to help you carry out a clinical audit in your practice. This uses a simple 1 (yes/meets criteria) or 0 (no/doesn't meet criteria) recording system - you might wish to add a 'partially meets criteria' measurement if you feel it is appropriate.

A simple action plan template to help you with implementing changes.

Record information and your reflections on any type of learning activity you do using this form.  You may find this especially helpful for activities you do that are valuable learning but maybe don’t earn CET points. 

You could upload the completed form to your MyGOC Personal Development Plan area, against a learning goal you’ve set, as evidence of non-CET activity towards meeting that goal. Or just keep a copy in your own learning portfolio if you have one.

You can use these forms to keep records of any patient consultation, prescribing activity or other CPD/learning activity you undertake.  The Summary version may be helpful if you wish to review a number of similar records and identify any overall patterns, observations, and learning from them.
