Fundus examination

How do you examine the fundus? As you know, it is a legal requirement to conduct an internal and external examination of the patient’s eyes whenever you do a sight test.

So far so good, but how do you do it? I was taught how to use the pen torch and loupe at university but I must confess that I don’t think I ever did it in practice, or if I did, it was so long ago I have forgotten about it! We were not taught how to use the Volk lens (yes, it was that long ago), so I had to teach myself that and – after quite a lot of practice – I now very rarely use my direct ophthalmoscope. So rarely, in fact, that I don’t take it out of my bag unless I have a patient who is unable or unwilling to reach the slit lamp. Long gone are the days of getting so close to the patient that you could feel them holding their breath whilst you did direct ophthalmoscopy!...
