Prescriptions and annual 'sight tests'

I have had several queries recently about whether optometrists can or should sign copy prescriptions for patients not seen by them. The scenario is that the patient has their eyes tested by optometrist A who (of course) gives them their prescription. The patient then returns to the practice some while later requesting a copy prescription and optometrist A is not there at the time. Several issues arise here.
