When you make a referral to the hospital eye services, the initial recipient is unlikely to be the same person who sees the patient at the appointment. The information you include in your referral needs to provide the required details for both the person arranging the appointment as well the ophthalmologist.

Of course, the person who deals with a referral initially will depend on the referral type and your local area; it may be an ophthalmologist, triage ophthalmic nurse, optometrist, GP or an administrator at a referral management service. This person only has the information you include on or with your referral letter and crucial clinical decisions will be based on its contents.
