The Falls and Bone Health Service is a specialist service which sits within Nottingham CityCare Partnership along with the Community Nursing and Rehabilitation teams, and Intermediate Care teams (Including Mental Health), and offers a city wide service.

The aims of the service are to offer specialist multi-disciplinary assessment and treatment to those people with complex multifactorial falls. We aim to reduce the numbers of falls and injuries caused by falls in older people through a range of multi-disciplinary interventions including; specialist physiotherapy, occupational therapy and nursing assessment – which includes medical assessment, medication review, continence assessment, postural blood pressure assessment, bone health assessment and addressing home hazards.

Interventions include advice and recommendations to GPs regarding medicines which contribute to falls, bone health/osteoporosis management, anxiety management, providing equipment and adaptations for the home, vestibular rehabilitation, and signposting on to other services and agencies.
