Alternaria is a filamentous fungus and a rare cause of atypical keratitis in humans. In this case report, we present a keratoconic patient who developed an Alternaria keratitis infection following a penetrating keratoplasty (PK). We discuss the risk factors, clinical characteristics, treatments and the use of polymerase chain reaction and in vivo confocal microscopy in the diagnosis of fungal keratitis.
