How would you deal with a patient who does not want to be referred?

Scenario

Colin Johnson, aged 51, visits you for his routine eye examination. He reports no unusual signs or symptoms but during the eye examination you notice a naevus which concerns you. You explain your concerns to Mr Johnson and say that you would like to refer him so that the naevus can be investigated. Mr Johnson is adamant that he does not want to be referred.
