As those of you local to me will know, I am Optometric Adviser to the Primary Care Trusts in Kent. Part of that work involves assessing so-called "contract compliance", namely whether contractors (and their performers) are complying with their General Ophthalmic Services contract. One of the ways in which this is done is to ask the contractors how they are ensuring that their records are "full, accurate and contemporaneous", as required by the contract.
